Cannabinoids in cancer treatment: Therapeutic potential and legislation

The plant Cannabis sativa L. has been used as an herbal remedy for centuries and is the most important source of phytocannabinoids. The endocannabinoid system (ECS) consists of receptors, endogenous ligands (endocannabinoids), and metabolizing enzymes, and plays an important role in different physiological and pathological processes.

Therapeutic Effects of Cannabis and Cannabinoids

Cannabis sativa has a long history as a medicinal plant, likely dating back more than two millennia (Russo et al., 2007). It was available as a licensed medicine in the United States for about a century before the American Medical Association removed it from the 12th edition of the U.S. Pharmacopeia (IOM, 1999). In 1985, pharmaceutical companies received approval to begin developing Δ9-tetrahydrocannabinol (THC) preparations—dronabinol and nabilone—for therapeutic use, and as a result, cannabinoids were reintroduced into the armamentarium of willing healthcare providers (Grotenhermen and Müller-Vahl, 2012).

Dr. Raphael Mechoulam, Cannabis and Cannabinoids Research Pioneer (November 5, 1930–March 9, 2023)

Raphael Mechoulam, the pioneering Israeli scientist known as the father of cannabis research, passed away at 92. In the 1960s, he identified the structures of CBD and THC, now widely used in medicine. Mechoulam’s team also discovered the endocannabinoid system, a vital network regulating immune function, memory, mood, pain, and psychosis, while mediating cannabis’s pharmacological effects.

The Therapeutic Potential of Purified Cannabidiol

The use of cannabidiol (CBD) for therapeutic purposes is receiving considerable attention, with speculation that CBD can be useful in a wide range of conditions. Only one product, a purified form of plant-derived CBD in solution (Epidiolex), is approved for the treatment of seizures in patients with Lennox-Gastaut syndrome, Dravet syndrome, or tuberous sclerosis complex.

Medical Cannabis for the Reduction of Opioid Dosage in the Treatment of Non-Cancer Chronic Pain

Medical cannabis (MC) is currently being used as an adjunct to opiates given its analgesic effects and potential to reduce opiate addiction. This review assessed if MC used in combination with opioids to treat non-cancer chronic pain would reduce opioid dosage.

Cannabis and Cancer: Unveiling the Potential of a Green Ally In Breast, Colorectal & Prostate Cancer

Cancer is a leading cause of death worldwide, with breast cancer (BC), prostate cancer (PC), and colorectal cancer (CRC) among the most prevalent. Cannabinoids, derived from Cannabis sativa, include cannabinol, delta-9-tetrahydrocannabinol (THC), and cannabidiol, which interact with the body’s endocannabinoid system, comprising receptors (CB1, CB2, GPR55) and enzymes. Emerging research highlights cannabinoids’ potential in cancer treatment, showing effects like apoptosis induction, proliferation inhibition, and reduced metastasis in BC, CRC, and PC, alongside anti-angiogenic and anti-inflammatory properties.
